If you are unable to sign up using a computer or a smart phone please call the Parish Office at (773) 545-8840
- We ask that you do not sign up or come to the church if you have a cough, or a temperature, or are not feeling well.
- People who are at risk for the Coronavirus, especially the elderly, and those with underlying or compromised medical conditions are encouraged to stay home for the time being.
- Please wear a mask at all times, sanitize your hands before entering the church,
and keep distance of 6 ft. - Please follow staff members instructions and follow all signs and markings.
By signing up, you acknowledge that you and your family members will fulfill the requirements mentioned above.

- Proszę nie zapisywać się na mszę lub przychodzić do kościoła z objawami choroby (kaszlem, podniesioną temperaturą).
- Osoby które są w grupie podniesionego ryzyka zachorowania na Coronavirus w szczególności ludzie starsi i osoby przewlekle chore proszone są o pozostanie w domu.
- Prosimy o odkażenie rąk przed wejściem do kościoła, podczas pobytu w kościele noszenie maseczki oraz zachowanie odstęu społecznego – minimum 6 ft. jest wymagane.
- Prosimy o przestrzeganie instrukcji podawanych przez osoby witające.
